All parents who serve as volunteers with access to students are required to comply with the following Diocesan Requirements:
1. The Safe Environment Program requires that any volunteer having contact with Carondelet students get a fingerprint clearance – this serves as a protection for both you and the students. Please fill out the LiveScan form (found here) and take it to a nearby location listed here for clearance. You will be responsible for the cost of the fingerprinting. Clearance will be submitted electronically to Carondelet.
NOTE: If you have done fingerprinting through the Oakland Diocese, you still have to complete this for Carondelet.
|2. The California Health and Safety Code (§121545) requires all school district volunteers to be examined and found free of communicable tuberculosis once every 4 years. Carondelet is required to keep an up-to-date certificate on file for each volunteer. Please have the Adult Tuberculosis (TB) Risk Assessment Questionnaire (found here) certified by your health care provider or provide us with a copy of a current negative TB test. TB test results are good for four years from the test date. Note that TB testing is done at the volunteer’s own expense. Details listed here.
|Note: If your risk factors change after the initial TB Risk Assessment Questionnaire was submitted, please have your health care provider complete another TB Risk Assessment Questionnaire and provide an updated copy to Angela Ricci: email@example.com.
| 3. Volunteers are required to take a Child Abuse Awareness training course in compliance with the Safe Environment Program at the Diocese of Oakland. If you have already taken this training within the last few years, you can use that certificate for volunteering at Carondelet. If not, you can access the on-line training by going to http://shieldthevulnerable.org/ and click on “Catholic Dioceses”, then select the “Oakland Diocese”, and find “Carondelet High School”. Select “ethical reporter”. Take the first class "Protect Children." The training takes about 90 minutes. Remember to print the certificate at the end of the training. You can scan and send it to: firstname.lastname@example.org. This certificate is good for three years.
| 4. Proof of insurance and a copy of a valid drivers’ license is required for all volunteers who drive students to/from events, with coverage limits equal to or greater than the minimum liability insurance required by the State of California, or $1,000,000 Combined Single Limit. Proof of insurance will need to be resubmitted upon expiration.
While you are volunteering at Carondelet, you are responsible for your own safety. Workers compensation benefits are not provided to volunteers.
